Gwen Polak, RN, NHA - Administrator

Gwen has over 30 years of experience as a nurse, Director of Nursing, Consultant and Administrator. She has worked at Clear Creek Care Center a total of 7 years.

Kay Stevens - Director of Nursing

Kay has been employed at Clear Creek for 26 years. She has held a variety of positions over the years; for the last 8 years she has been the Director of Nursing. She is a Geriatric Nurse Practioner whose experience, compassion and commitment provide leadership for the nursing staff.
